This FINRA EFS Broker-Dealer Authorization Form must be signed by a principal of your company (usually your Compliance Officer at the home office) and mailed to FINRA before fingerprints can be accepted electronically. Please download this form and forward it to the appropriate individual. (It is a fill-in PDF form, you may type in the blanks, print, then sign.)
